常駐模式與顯式命令
常駐模式適合日常定位；需要精準路徑查詢時，用 /graphify query、/graphify path、/graphify explain 直接讀 graph.json。

graphify hook install：commit/checkout 後自動重建。/graphify ./raw --watch：背景監聽變更。
